This is a REGISTRATION FORM to sign up your child as our Autsome Member. All information is required to be completed before you collect your Autsome Kit.

Only children who are diagnosed with Autism Syndrome Disorder (ASD) is applicable to register.

Once this online registration is completed, kindly collect your Autsome Kit at Sunway Carnival Mall's Concierge, 2nd Floor.

As an Autsome Member, your child may enjoy the privileges (only applicable on every Tuesday*) below:
- Shopping Privileges
- Priority Lane at participating outlets

Autsome Facilities (Available Daily):
- Calm Rooms (2nd Floor, next to Old Town White Cofee)
- Sensory Wall (2nd Floor, next to Mr. Toy)
